Sunday-Thursday - 7am-3pm - Full Time -

Job Summary: The Case Manager is primarily responsible for providing recovery-oriented supportive counseling, individual and group, to residents with varying levels of impairment from substance use and mental health disorders, and develop and implement therapeutic treatment plans in the framework of recovery, addressing both substance use and mental health symptoms. The Case Manager contributes to Bay Cove's mission by partnering with individuals in their recovery journey and improved quality of life.

This is a non-exempt position.

Essential Functions of the Position:

  • Interact therapeutically with residents to ensure their needs are met.
  • Maintain resident records in compliance with agency and funder requirements.
  • Complete high quality assessments and treatment plans for assigned residents within expected timeframes.
  • Provide at least five hours of evidence-based clinical treatment programming each week and provide direct group and individual clinical treatment to residents via evidence-based practices.
  • Provide coordination of all required treatment services either directly by the program or through Qualified Service Organizations, Business Associate Agreement, or other applicable agreement.
  • Planning for resident's completion of treatment, and identification of transitional, discharge and aftercare supports the resident.
  • Family support services, including family therapy, or social or education services.
  • Provide daily programing that facilitate the stabilization of the residents' substance use and mental health symptoms, maintenance of medication regimens, participation in community interactions and promoting residents' recovery
  • Provide timely and accurate communication and coordination of care with other providers, such as DCF, Probation, and DPH
  • Provide individual counseling to all clients regarding substance abuse.
  • Provide group counseling to all clients regarding substance abuse. Groups can include educational, life skills, relapse prevention groups.
  • Provide crisis intervention when necessary.
  • Provide case management and engage in care consultation regarding specific clients with other staff, relatives and appropriate representatives of other agencies, including social service agencies, housing authorities, health care providers and vocational issues.
  • Complete progress notes and group notes
  • Enforce policies and procedures of the program.
  • Assist with the medication administration process by providing oversight, storage, and coordination of self-administration of medication, including:
  • Conduct urine drug screens in a trauma informed manner and document and report all outcomes to the team.
  • Attend and participate in staff meetings, supervision, and related professional development sessions.

Job Requirements:

  • BA/BS or minimum four years' experience in substance use and behavioral health
  • CADAC/LADAC or equivalent preferred, or actively pursuing certification.
  • Experience in individual and group counseling.
  • Prior knowledge of the field of substance abuse.
  • Knowledge of modality specific issues, such as substance abuse, homelessness, trauma, and other co-occurring disorders.
  • Must be knowledgeable with the Confidentiality of Alcohol and Drug Abuse Patient Record (Title 42CFR-Part 2)
  • CPR/First Aid Certification
  • Must stay current with any modality required training, including but not limited to HIV/AIDS, Hep C, Narcan, Servsafe, Crisis Intervention, Harrassment, etc.
